Key Responsibilities
As a Home Health Nurse - Case Manager at Amedisys, your primary responsibilities will include:
- Patient Assessments: Perform comprehensive patient assessments and collaborate with the care team to develop and implement a plan of care.
- Care Planning: Develop and implement individualized care plans, taking into account the unique needs and goals of each patient.

Qualifications
