Role Description The Clinical Pharmacist – Value-Based Care plays a key role in a pharmacy program focused on improving medication adherence through data-driven outreach and collaboration. This remote position supports a small, specialized team dedicated to adherence initiatives, working closely with pharmacy technicians, payers, and Value-Based Care leadership to identify and address gaps in therapy. • Using payer-provided adherence reporting, the pharmacist reviews non-adherence data. • Supports technician-led patient outreach. • Serves as the clinical escalation point when barriers require pharmacist-level intervention. • Provides operational and program leadership support, including participation in payer meetings, workflow development, training initiatives, and project management tied to Value-Based Care strategies. This position offers a unique opportunity to work at the intersection of clinical pharmacy, payer collaboration, and operational program management, with a balanced focus on patient support and program oversight. This is a remote role. A caregiver in this position will work Monday through Friday with 1-2 onsite requirements per year. A caregiver who excels in this role will: • Provide safe, efficient, patient-centered and evidence-based pharmacy and medication services. • Ensure the appropriate selection, dosing, administration and dispensing of medication orders. • Verify medication orders and/or dispense medications accurately according to state and federal regulations. • Document services in a complete, accurate and timely manner. • Actively contribute to medication safety initiatives. • Ensure all pharmacy operations are in compliance with national accreditation standards. • Educate and train students, residents, other caregivers, and/or patients. • Provide cost-effective care. Qualifications • PharmD or Bachelor’s of Science in Pharmacy from an accredited College of Pharmacy • Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire • Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) • Completion of a Board of Pharmacy approved course in the administration of immunizations or certification within 90 days of hire for Individuals administering immunizations • Current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certifications through the American Heart Association (AHA) upon or within one of hire for individuals responding to adult or pediatric cardiopulmonary arrest events Requirements • Board Certification • Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience • Post-Graduate Year 2 Residency (PCY2) or relevant experience for Clinical Pharmacists in a specialized pharmacy practice area • Experience in value-based care • Prior work with a payer (ex: UnitedHealthcare, Humana, etc.) • Retail pharmacy experience Physical Requirements • Ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time. • Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs. • Position often includes reaching, stretching, stooping, climbing steps and sitting. • Must have normal or corrected vision. • Must have ability to clearly communicate verbally by phone or in person.
