What We’re Looking For Trillium Health Resources has a career opening for an In Reach Coach to join our team! The In Reach Coach is responsible for Diversion and In Reach activities for the TCL Special Healthcare Population with Serious and Persistent Mental Illness (SPMI) residing in Adult Care Homes, State Hospitals, Correctional Institutions and various community settings by providing education about community resources, services, supports and TCL permanent supportive housing. The In Reach Coach ensures members receive whole person care focused on all members’ needs including physical health, behavioral health, and social determinants of health/unmet health resource needs that helps to establish positive outcomes for members and facilitate community integration. On a typical day, you might: - Engage members who meet criteria for TCL special needs population to provide education about permanent supportive housing, supports, services, and other community resources. - Partner with Diversion staff, Transition Coordinators, and Next Step Rehousing Team to facilitate transition planning and progression through the TCL process for those members who desire permanent supportive housing in the community. - Collaborate with Trillium Integrated Care Nurse, OT, COTA and Clinician to identify complex care needs and clinical service linkage needs.  - Facilitate community integration, link to services and provide education and support for those members who choose not to transition to permanent supportive housing in the community.  - Utilize person-centered planning methods/strategies to gather information, to get to know the members and legally responsible persons to adequately address specific needs of the individual. - Provide education and support to members and guardians in learning about and exercising rights and responsibilities. That’s why we offer: - Typical working hours: 8:30 am – 5:00 pm; flexible work schedules with some roles with management approval. - Work-from-home options available for most positions - Health Insurance with no premium for employee coverage - Flexible Spending Accounts - 24 days of Paid Time Off (PTO) plus 12 paid holidays in your first year - NC Local Government Retirement Pension (defined-benefit plan) https://www.myncretirement.gov/systems-funds/local-governmental-employees-retirement-system-lgers/lgers-handbook - 401k with 5% employer match and immediate vesting - Public Service Loan Forgiveness (PSLF) qualifying employer - Quarterly stipend for remote work supplies Qualifications Required: - High School Diploma or GED required. - Be a person with (1) year lived experience with a behavioral health condition and/or substance use.- *Lived Experience is defined as living in recovery with mental illness and/or substance use disorder. - The following immunizations are required for this position:  - Fully vaccinated against Covid-19, Influenza (annually within Influenza season Nov 1 – March 31), MMR, Varicella and Tdap (Tetanus, diphtheria pertussis).  - MMR and Varicella - must show proof of vaccination or have blood drawn to show immunity.  - Tdap - must show proof of one dose.  - Employees must provide proof of all immunizations prior to their start date or agree to provide proof of all immunizations within 4 months of start date. - Required to be a North Carolina Certified Peer Support Specialist, which may be attained within six (6) months of commencing employment with Trillium.- The NCCPSS Program requires all applicants for certification to meet the following Program eligibility requirements:- Must be 18 years or older - Have lived experiences and in recovery from a serious mental illness and/or substance use disorder - Must be in recovery for at least ONE year  - Have a high school diploma or equivalent - Must have a valid driver’s license. - Must reside in North Carolina and within 75 miles of Wayne County.
