Key Responsibilities:
- Develop and implement personalized Plans of Care (POCs) tailored to patients' and families' unique needs, updating them as necessary to reflect changes in requirements.
- Collaborate with internal and external stakeholders to ensure provider-related activities align with organizational goals and objectives.
- Maintain compliance with Utilization Review and regulatory requirements by identifying trends and making process changes as needed.
- Support the medical staff quality review process by identifying quality issues and promoting efficient planning for patient and family needs.
- Utilize accurate and comprehensive documentation practices to communicate effectively with healthcare professionals, patients, and families.
- Coordinate interdisciplinary team huddles/communication, identifying and communicating patients' healthcare needs based on best practice standards.
- Evaluate current treatment plans to identify barriers and establish goals, working collaboratively with support services departments to ensure successful transitions during discharge.
- Serve as a visible resource on the unit, providing guidance and support to patients, families, and healthcare teams.
- Stay up-to-date on the latest developments in reimbursement procedures, managed care contracts, and patient status.
Requirements and Qualifications:
Essential Qualifications:
- Completion of a nursing education program approved by a board of nursing.
- Registered Nurse (RN) licensure with the South Dakota Board of Nursing.
Preferred Qualifications:
- Bachelor's degree in Nursing.
- 5+ years of acute care experience and 5+ years of Registered Nurse experience.
- Commission for Case Management Certification (CCMC) accreditation from an accredited university or training professional.
