Slate Health is expanding our Chronic Care Management (CCM) team and is looking for compassionate and organized Licensed Practical Nurses (LPNs) who enjoy helping patients with chronic medical conditions.
This position offers a hybrid work structure, allowing you to work primarily from home while also participating in occasional in-office collaboration or training in Florida.
We are open to both part-time and full-time candidates depending on availability and fit.
You will work with a clinical team supporting patients between their doctor visits, helping them stay on track with their care plans and overall health.
What you'll do:
· Speak with patients and caregivers by phone
· Help patients stay on track with medications and care plans
· Coordinate with physicians and nurse practitioners
· Document patient interactions
· Support patients managing chronic conditions
Requirements:
· Active LPN license in good standing
· Comfortable using basic computer systems and documentation
· Strong communication and organization skills
· Reliable home office setup
· Ability to attend occasional in-person meetings or training in Florida
