Human Touch Home Health is seeking an experienced Physical Therapist (PT) to provide skilled in-home therapy services in King George and Massaponax, Virginia.
We are currently hiring for PRN positions based on your availability and preference.

What You’ll Do
- Perform comprehensive in-home PT evaluations and reassessments
- Develop and implement individualized treatment plans per physician orders
- Provide skilled therapy to improve mobility, strength, balance, and function
- Complete accurate and timely OASIS documentation
- Educate patients and caregivers on home exercise programs and safety
- Collaborate with physicians and interdisciplinary team members
- Supervise Physical Therapist Assistants (PTAs) as applicable
What We’re Looking For
- Active Virginia Physical Therapist license
- Minimum 1–2 years PT experience (home health preferred)
- OASIS experience preferred
- EMR experience (WellSky/Kinnser or Homecare Homebase a plus)
- Strong clinical and documentation skills
- Valid driver’s license & reliable transportation
