As one of the nation’s few fully integrated healthcare organizations, APH delivers comprehensive, coordinated medical and social support through a wide range of specialized service lines.  With continued growth on the horizon, APH is seeking mission-driven individuals who are passionate about improving health outcomes and supporting those in need.  POSITION PURPOSE  The Registered Dietitian (RD) provides clinical nutrition services, including nutrition assessment, modified diet formulation, self-manage training, and nutrition intervention to members. As the RD, you will be responsible for a variety of tasks which includes, but are not limited to, improving the quality of life and health literacy of members, promoting effective healthcare utilization, and monitoring healthcare resources to achieve optimal outcomes for members. The RD assists in the coordination of care and services of selected member populations across the continuum of illness and promotes the effective utilization of healthcare resources to ensure member satisfaction and quality care. This position operates in a remote capacity with normal work hours, Monday-Friday, 8a-5p.  DUTIES & RESPONSIBILITIES - Evaluates members for case management services and determine the appropriate level of  - Assess the nutritional status and provides nutrition intervention to members, including diet education. - Screen members for nutritional risk and malnutrition. - Develop dietary plans of care. - Provide nutrition consults/assessments as requested by team members. - Coordinate nutritional care with other care team members including CMs, SW, NPs, and members assigned to a PCP. - Use clinical nutrition standards of care to assess patient nutritional needs. - Educate and support members’ needs, taking into account patients’ physical or age-related needs. - Accurately chart all assessments, findings, the education given, care plan, and outcomes in the patient’s electronic chart. - Collaborate with other departments to ensure members’ needs are met. - Participates in Interdisciplinary Care Team meetings, as needed. - Develop positive relationships with clients, caregivers, providers, and other team members. - Ensures quality, effectiveness, and efficiency in case management activities. - Complies with all HIPAA regulations. - This job description in no way states or implies that these are the only duties to be performed by this employee.
