River Terrace Retirement Community is seeking a passionate and detail-oriented Registered Nurse Assessment Coordinator (RNAC / MDS Coordinator) to join our skilled nursing team in Bluffton, Indiana. This full-time role offers the opportunity to make a meaningful impact by ensuring high-quality resident care through accurate assessments, regulatory compliance, and effective care planning.
Your Role & Responsibilities
As the RNAC / MDS Coordinator, you will:
• Conduct routine resident assessments according to schedule and as needed for condition changes, hospital discharges, and other clinical events
• Accurately observe, assess, and communicate changes in resident condition to appropriate interdisciplinary team members
• Collaborate with Social Services, Activities, Dietary, and Nursing to ensure timely completion of assessments and Care Area Assessments (CAAs) with appropriate documentation
• Complete MDS sections, quarterly reviews, CAAs, and care plans in compliance with state and federal regulations
• Develop, update, and monitor individualized resident care plans based on assessment findings
• Prepare and distribute the MDS schedule to the interdisciplinary team
• Transmit MDS data to the state weekly (or as required) and maintain organized, accurate records
What We’re Looking For
• Registered Nurse (RN) license preferred; LPNs with strong MDS experience will be considered
• Minimum of 5 years of experience as an RNAC / MDS Coordinator
• Prior experience working as a Coordinator in a skilled nursing or long-term care setting
• Strong knowledge of MDS processes, assessment timelines, CAAs, and care planning requirements
