Make a Difference as a Hospice RN Case Manager
Hospice of Lenawee is hiring a compassionate Registered Nurse (RN) Case Manager to provide high-quality, patient-centered hospice care in patients’ homes throughout our 35-mile service area which includes Lenawee County and surrounding areas.

Schedule
- Full-time (40 hours/week)
- Day shift
- Limited weekend rotation
- Holiday rotation (3–4 per year)
Requirements
RN Case Manager Responsibilities
- Provide skilled nursing care to hospice patients in their homes
- Manage a caseload of patients and families
- Develop and coordinate individualized care plans
- Monitor and manage pain and symptoms
- Educate and support patients and caregivers
- Collaborate with physicians and interdisciplinary team members
- Document care in accordance with regulatory standards
Qualifications
Required:
- Active Michigan RN license
- Graduate of an accredited nursing program
- CPR certification
- Valid driver’s license and insured vehicle
- Ability to travel locally within Lenawee County and surrounding areas within our 35-mile service area
Preferred:
- BSN
- Experience in hospice, home health, oncology, or case management
- Experience providing end-of-life care
