Job Overview:
We are seeking a compassionate and skilled Health Aide or CNA to join our team at Fallon Health's Summit ElderCare in Webster, MA. As a Health Aide or CNA, you will play a vital role in providing personalized care and support to our participants, helping them to maintain their independence and quality of life.

Responsibilities:
- Provide personal care, light housekeeping, and assistance with activities of daily living (ADLs) to participants in their homes and at the PACE center
- Collaborate with the interdisciplinary team to develop and implement care plans
- Carry out non-skilled treatments, including vital signs, transfers, and toileting
- Maintain a clean and safe working environment and assist with meal preparation and serving
- Provide accurate and timely documentation in the EMR and other systems
Requirements:
- High school graduate or equivalent
- Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program preferred
- Certification as a Home Health Aide or Nursing Assistant; CPR and First Aid Certification or willingness to be certified within 60 days of employment
- One or more years of experience working with the elderly in a healthcare setting
- Reliable transportation required
